Understanding your coverage and getting the most out of your benefits doesn’t have to be complicated. While we are not a contracted, “in-network” provider for any third-party insurance company, we accept coverage benefits remitted to us from all major dental plans. To simplify and streamline your experience, we welcome the opportunity to:
- Verify your insurance in advance to confirm there are benefits available to you
- Explain your benefits plus any out-of-pocket costs specific to the services and procedures we are recommending for you
- File an “out-of-network” claim with your provider on your behalf when services are rendered
To get this process started, we need your policy information and your date of birth. You can share this information with us via email – hello@glazerdentistry.com – or by calling the office at (504) 895-1137.
For your convenience (and our own), we strive to offer the most accurate estimate of your benefits plus any additional co-pays, deductibles or out-of-pocket costs that will be due at the time of service. If estimates turn out to be incorrect, patients are responsible for any fees not covered by their insurance provider.
