If this is your first visit to our office, welcome! To save you time and allow us to serve you better, you may choose to download our patient intake forms and complete them in advance.
If you prefer to fill them out in office, please arrive 15 minutes before your appointment time so we can review your information and answer any questions you may have.
Our office accepts payment by cash, personal check, and all major credit cards. We also accept out-of-network benefit payments from all major dental insurance plans.
If you do not have dental insurance, don’t worry – many of our patients do not have third-party coverage. But whether you have benefits to discuss or not, ask us anything! We answer questions about fees, billing, or any other financial considerations with respect, discretion, and transparency.
